1954 నిర్దేశాల ప్రకారం 1 3⁄8 ఇంచుల (35మిల్లీ మీటర్ల) వ్యాసార్ధం కలిగిన వృత్తాకార బంగారు పతకాన్ని ఈ పురస్కార సమయంలో బహూకరిస్తారు. పతకం ముఖభాగంలో సూర్యుని బొమ్మ ఉండి, కింది భాగంలో వెండితో "భారత రత్న" అని దేవనాగరి లిపిలో రాసి ఉంటుంది. వెనకవైపు మధ్యభాగంలో ప్లాటినం లోహంలో భారత చిహ్నం, కింది భాగంలో వెండితో భారత జాతీయ నినాదం "సత్యమేవ జయతే" అని రాసి ఉంటుంది.<ref name="award1"/>
