    • The rules for old files are explained at Commons:COM:GOF and those rules are based on the history of some page on Commons. I don't know whether English Wikipedia uses the same rules as Commons. According to Commons, "Prior to 8 January 2006, it was only required that the file was properly licensed and attributed, with no specific statement of permission being required on the image page." However, in this case, the permission from the copyright holder is stated on the file information page, so I would assume that the permission is exactly as stated. Between 8 January and 15 March 2006, the uploaders were required to quote the permission e-mail on the file information page, which is essentially what the uploader has done here, although the file was uploaded earlier.
According to Twinkle's documentation for WP:CSD#F3, {{db-f3}} doesn't apply to images uploaded prior to 19 May 2005, but this date isn't mentioned at WP:CSD#F3, so maybe it is obsolete information. On that date, User:Jimbo Wales sent this e-mail to a mailing list, stating that non-commercial and "by permission only" licences no longer are accepted, and this is probably why Twinkle mentions that date. According to that e-mail, we still don't want earlier files under those licences, so I would still assume that such files are deleted, although they might need a deletion discussion. Curiously, the e-mail doesn't mention nonderivative licences at all.

I believe DryMartini has uploaded this file and released it under a GFDL license in good faith. As he has stated (perhaps somewhat confusedly), this image was created as part of a university group assignment.
